After
you have reviewed the Agent Licensing
Procedures Form, you will need to complete the Agent
Appointment Form and Appointment
Data Sheet. Mutual of Omaha will conduct a background
check, please review the Fair
Credit Disclosure Sheet and complete the Background
Information sheet. If you choose to have your commission
deposited directly into your bank account please complete
the Direct Deposit
Form.
